Functional and Aesthetic Jaw Surgery

Dr. Benjamin Subtelny founded Subtelny Face & Jaw as a surgical practice dedicated to corrective jaw and TMJ surgery, with each operation carefully planned to achieve aesthetic facial balance, an open airway, and a healthy bite built on stable jaw joints. Approach

Form Follows Function

The bones of the face set its shape. The same movements that correct a bite or open an airway also set the profile, the chin, the jawline, the lips, and the neck. Correcting the bite is the starting point of jaw surgery. Dr. Subtelny plans every operation to achieve far more: a balanced, natural face, a beautiful smile, and an open airway, all designed together in one three-dimensional plan.

The planned position, shown dashed, corrects the bite, carries the lower lip and chin forward, and sharpens the angle between the jawline and the neck. Cephalometric measurements help confirm a diagnosis, but each plan is built from the patient's own face, not from population averages.

The Face

Facial proportions, profile, how much tooth shows at rest and in a smile, lip support, chin projection, and the definition of the jawline and neck are measured and planned to the millimeter. Correct skeletal support gives the skin and soft tissues a lasting framework that looks balanced and natural today, while fighting the effects of gravity through aging.

The Airway

Jaw surgery is always an airway procedure, even for patients without sleep apnea, because the tongue and soft palate move with the jaws. Every plan accounts for its effect on breathing.

The Bite

Jaws positioned so the teeth fit together, with your orthodontist refining the final details. This is the least jaw surgery should achieve. A bite can be corrected at the expense of the airway or the jawline, for example by setting the lower jaw back alone. Dr. Subtelny designs every plan so the bite, the face, and the airway improve together, and positions the jaws to create a naturally beautiful smile.

The surgeon

Benjamin Subtelny, DDS

Dr. Subtelny completed his residency in oral and maxillofacial surgery at St. Joseph's University Medical Center in Paterson, New Jersey. He chose St. Joseph's because its high volume of facial trauma is the proving ground for a confident, effective facial surgeon. He then completed subspecialty fellowship training in complex orthognathic, TMJ, and craniomaxillofacial surgery. During fellowship, he performed hundreds of operations in corrective jaw surgery, TMJ surgery, facial trauma, and reconstruction.

For Dr. Subtelny, that volume of cases in a short time developed the artistic eye that aesthetic and functional results demand. Treating cases across the full range of facial deformity taught him which combination of movements best serves a face. The operation itself became second nature, while the artistry in planning that drives beautiful, predictable results continues to make the work invigorating.

He is board-certified by the American Board of Oral and Maxillofacial Surgery.

Orthodontics

Coordinated With Your Orthodontist

Corrective jaw surgery is a partnership between surgeon and orthodontist. Dr. Subtelny works with your orthodontist at every stage, so tooth movement and jaw surgery fit together as one comprehensive treatment.

Before Braces

The surgical goals and the tooth movements needed to reach them are agreed upon with your orthodontist before treatment begins.

Before Surgery

Dr. Subtelny plans every three-dimensional movement of the jaws on a computer ahead of time. That plan produces the splints, guides, and implants that allow for precision in the operating room. While every plan is tailored to the patient's goals, the process behind it is the same every time, and your orthodontist is welcomed throughout.

After Surgery

Your orthodontist receives updates at each milestone, and you return to them to finish treatment.

Process

How a Case Is Planned

Jaw and facial surgery is decided long before the operating room. Every case moves through the same five stages, and Dr. Subtelny is the surgeon at each one.

  1. Consultation and Facial Analysis

A detailed analysis of the face and its soft tissues, at rest and in animation, along with the teeth, jaws, joints, and airway. Records include facial measurements, clinical photographs, a cone-beam CT scan, and digital scans of the teeth.

  2. Coordination

Goals and timing are agreed upon with your orthodontist, dentist, or sleep physician before anything is scheduled.

  3. Virtual Surgical Planning

Dr. Subtelny plans every movement himself, in three dimensions on your own scan. Each is tailored to your goals and your soft tissue drape, and measured in millimeters for a natural result.

  4. Surgery

Performed under general anesthesia at a hospital where Dr. Subtelny holds privileges, by the surgeon who planned it.

  5. Recovery and Follow-Up

Scheduled visits through bone healing and beyond, with updates sent to your referring doctor at each stage.
